Due to Federal health Care Reform, individuals whose health insurance coverage ended, or who were denied coverage, or were not eligible for coverage, because of the availability of dependent coverage of children before attainment of age 26 are eligible to enroll in the City’s of Everett’s Health Plans. Individuals may request enrollment for such children 30 days of this notice (April 27, 2011). Enrollment will be effective July 1, 2011. Eligibility is only for children who are not eligible for group coverage on their own, or through their spouse. You will be required to sign an affidavit.
